Insurance Information

We make every effort to provide you with the highest quality of care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. If we participate with your plan, we will file the claim for you. You are responsible for co-pays/co-insurance, deductibles and non-covered services at the time of service. If we do not participate with your insurance plan, we will still file a claim to your insurance for you. However, you remain financially responsible for all charges incurred.

Note: We do not file claims with liability/accident insurance carriers.

Written authorization must be obtained from your employer before treating a Workers Compensation injury. Claims for work related injuries are sent to the insurance company or your employer. If not covered by insurance, you agree to be responsible for the balance due for the services rendered. Please call if you have any questions or concerns regarding your initial visit.
